My Life, My Future (MLMF) has been developed by Transitions Lead at Birmingham Womens and Childrens Hospital. MLMF is a person-centred, transition planning tool, for young people from age 11+, who are living with long term health conditions. It can be used to consult with disabled young people or those who find it harder to speak up for themselves. MLMF has been updated and reviewed several times and now includes resources from Alstrom Syndrome UK (ASUK), Transition Knowledge and Skills (T-KASH) logos.
The Healthier Together programme relies upon patients and healthcare professionals working together to improve how local healthcare is delivered. The website provides advice for parents, young people and pregnant women, and clinical resources to support healthcare professionals – which means that your child is likely to receive consistently high-quality care, irrespective of which healthcare professional they see.
