Growing Healthy 0-25 County Durham

A range of health professionals who aim to work in partnership with children, young people and their families to promote their health and wellbeing.

  • Our Family Health Visitors provide care until the end of Foundation Stage (Reception Year in school) and beyond when actively working with a family.
  • Our Family Health Visiting Team offer ten routine Healthy Child Programme contacts up to the end of reception year in school, including to children not in education settings. This is three more than we currently offer.
  • Well Baby Clinics take whole family approach of Community Based One Stop Shops, developed with service users and partners.
  • Health Exercise and Nutrition in the Really Young (HENRY) programme to support a healthy start and brighter future for families.

0-5 Children’s Service

All families with a child under five in Darlington have access to a Growing Healthy 0-5 Team consisting of health visitors, early years practitioners and infant feeding specialists. They provide expert health assessments, advice, support and interventions for babies, children and families.

The team encourages and supports parents and carers to develop life-long skills to enable them to make informed choices that affect their family’s future health and wellbeing.

The team is community-based and is accessible at a range of different venues including the family home, children’s centres and health centres. The service works in partnership with other agencies to ensure all families receive support.

Service Information 

The Growing Healthy Gateshead 0-19 Healthy Child Programme service is an integrated service for families, expectant mothers, children and young people that offers interventions from the pre-natal stage up to the age of 19. The Trust leads on the delivery of this service, with a focus on working across services and organisational boundaries to improve public health outcomes.

The universal reach of the HCP provides an invaluable opportunity from early in a child’s life to identify families that are in need of additional support and children who are at risk of poor outcomes.

The aims of the HCP service is to:

  • Help parents develop and sustain a strong bond with children;
  • Encourage care that keeps children healthy and safe;
  • Protect children from serious disease, through screening and immunisation;
  • Reduce childhood obesity by promoting healthy eating and physical activity;
  • Identify health issues early, so support can be provided in a timely manner;
  • Make sure children are prepared for and supported in all child care, early years education settings and especially are supported to be ready to learn at age two and ready for school by age five.

Within Children's Services we have dedicated teams working to support you and your family on a formal and informal basis.  We understand that most Families experience challenging times, and we are here to support you.

Early Help teams will support you in identifying where a child and their family may need extra support in either the early years of a child's life or in the early stages of a problem at any time in their childhood.  Early Help services work with families with children aged 0-19 (or 25 with additional or complex needs).

Early Help is split into many services, such as:

0-5 Children’s Service

The health visiting and school health service works with families to support children and young people's development (ages 0 to 19), providing additional support, or information to help people find more support, when it is needed.

We aim to provide a health support service for children, young people and their families in all areas of their lives. 

The services we provide are for children and young people from age 0 to 19. They include:

Service Information 

The Health Visiting service is made up of a team of specialist nurses who offer advice and guidance to parents with children aged 0 to 5 years old.

We offer a wide range of support and advice on things like:

  • baby clinics
  • behaviour
  • growth and development
  • immunisations
  • minor ailments
  • nutrition
  • support groups

We can also connect you with other agencies if you need advice or support around housing, benefits or debt.

We’ll support you and your family until your child goes to school. We do this through a national scheme called the Healthy Child Programme.

The programme includes a number of ‘reviews’, during which we’ll talk about how your child is doing and discuss any concerns you have. These reviews might take place in your home, a child development clinic or a children’s centre.

We work closely with other health professionals and if appropriate we might refer you and your child to other specialist services.

Our 0-19 service consists of health visitors, school nurses, nursery nurses , staff nurse’s,  health care assistants and school nursing assistants.  

The 0-19 team work as part of wider community teams with GP, midwives, Children’s Centres, Paediatricians, Speech Therapists, Nurseries, Schools, Sexual Health Services to provide care and support to promote children’s’ well-being and prevention of harm.

Our 0-5 team offer home visits to all families in the community of South Tyneside, from pregnancy through to school entry, as recommended by the national Child Health Promotion Programme. If you’re a parent, the team will support you with your own health needs and will help you develop a healthy lifestyle. They also provide ‘drop in’ baby clinics and tailor support to your child’s individual and family needs. Every child has a named health visitor until they reach school entry.

Growing Healthy Sunderland provides an integrated Public Health service for expectant mothers, children and young people and their families in the city. This includes:

  • Provision of antenatal reviews from 28 weeks of pregnancy.
  • Provision of all mandatory universal reviews.
  • Provision of a 3-4 month review.
  • Provision of school readiness review.
  • Targeted support for teenage parents via the implementation of the Family Nurse Partnership.

  • Audiology Screening at commencement of primary education.

  • Provision of enhanced support to children and young people with higher levels of need.
